Membership forms

Depending on your profile, choose here below from the downloadable membership forms:

 

For new membership applications:
Please attach the following to your Membership Form:

  • your CV and Letter of Intent (We can provide you a template for this, upon request)
  • your application form for membership in an International Committee of your choice. It is a must to join an International Committee.  (downloadable form here )

For retired membership applications:
Applicants should provide evidence of their former status as a museum professional.
For student membership applications: 
The student must be enrolled in a program of study leading to a postgraduate degree in museology, museum studies, museum education, museum management, conservation, or equivalent museum-related subject;

The application must be made to the National Committee of the country in which the student is enrolled in a nationally recognized institution, and include a copy of registration or a letter from the faculty administrator verifying enrollment and curriculum of study unless otherwise authorized by the host National Committee.

Student membership is subject to the review and approval of the host National Committee.

Your application is subject to the approval of ICOM Philippines’ Board of Trustees. Once it is approved, we will notify you to pay your membership fees. When we receive payment of your membership fee, we will forward your application to ICOM Secretariat in Paris who issues the ICOM membership card. We will inform you at the soonest possible time when we receive the membership card from ICOM Secretariat.

 

2) Membership Fee

Individual Membership Categories

Regular (Voting)

PHP 2,200.00

Retired Professional (Voting)

PHP 2,100.00

Student (Non-voting)

Students must supply proof of full time course enrollment with application

PHP 1,500.00

Institutional Membership Categories 

 

All categories (one rate for 2012. 2013 rates to be announced soon.)

PHP 15,000.00

ICOM membership is annual and runs from 1 January to 31 December of the year for which subscriptions are paid.

 

3) Payment

You may pay your membership fee at the following payment centers:

 a. For Visayas and Mindanao applicants: ICOM Philippines Secretariat, University of San Carlos University Museum, USC Campus, P. del Rosario St., Cebu City 6000 – through Malou Samson.

b. For Luzon applicants: IMDM Office, 4/F ACCM, AIM Campus, Benavidez cor. Trasierra Sts., Legaspi Village, Makati City, 1229 Metro Manila – through Gina Barte. 

Once we receive your payment, you will be issued a temporary receipt which will be replaced with an official receipt once we have completed our BIR registration and obtained authorization to print official receipts.
